Hemorrhoid Treatment Center of Florida Investigates New Therapy for External Hemorrhoids

Dr. David Shapiro, Director of Hemorrhoid Treatment Center of Florida (HTCF), recently began an investigation into the use of Low Level Laser Therapy (LLLT) after a hemorrhoid procedure. Emerging literature has documented the effectiveness of LLLT in promoting the early healing of various wounds in laboratory studies.

Dr. Shapiro formerly played a role in the development of another procedure called Ultroid® treatment. However, the painless Ultroid procedure is only appropriate for the treatment of internal hemorrhoids. External hemorrhoids require a minor surgical procedure, which is performed using local anesthesia in a medical office. Discomfort and swelling after resection (surgical removal) lasts several days, and is managed with medication that allows for normal activities.

LLLT addresses the discomfort and recovery common with external hemorrhoid treatment. The therapy currently investigated by HTCF is performed in the recovery period after the resection.

Lasers have been used to reduce pain and stimulate positive outcomes for many years. In 1967, a Hungarian researcher demonstrated, through testing on model mice, that laser energy could stimulate hair growth. This was the first demonstration of a laser bio stimulation, which is still used today by physical therapists around the world.

The positive effects of laser use shown on animal models include a decrease of inflammation and swelling, and the promotion of wound healing. Even so, because of the biochemical mechanisms that result, these effects are not completely understood. The complexity of the parameters that are used to measure the laser dosage in publications are often not in agreement.

However, it is generally agreed that cellular elements called chromophores are responsible for the effect of the laser light on animal cells, resulting in changes in the inflammatory response by altering the cytokines. Edema and pain decrease because of this diminished inflammatory reaction. Fibroblasts, which are a fundamental element in wound healing, are also stimulated.

Therefore, a basis for Low Level Laser Therapy positively effecting wound healing exists. There has never before been a study to prove the beneficial effect following external hemorrhoid removal, which is the basis of HTCF conducting this investigation.
