To perform the vasectomy reversal procedure, Dr. Hickman will do a microsurgical repair of the vas deferens with the aid of a powerful operating microscope and fine nylon suture. The primary purpose is to make a direct and careful alignment of the lumen of the two ends of the vas deferens.

There are three possible scenarios after examining the sample fluid. The best possible scenario is that the fluid contains whole sperm. The second possibility, which is more common, is that the fluid is thin and does not contain whole sperm but instead sperm parts. The third scenario is that the fluid is pasty and thick and does not have any sperm at all. This last scenario usually indicates the presence of an obstruction, which is an epididymal blowout. This means that in addition to the vasovasostomy, Dr. Hickman will also need to perform a vasoepididymostomy. To optimize the possibility for a successful outcome, both procedures have to be done. To perform the vasoepididymostomy, Dr. Hickman will need to do a microscopic invagination of the epididymal tube into the lumen of the vas. But it should be noted that there will be no additional cost for the patient even if both repairs have to be performed.
