Maryland Self-Directed In-Home Nursing For The Developmentally Disabled Updated

With this update, the organization aims to provide comprehensive support to Maryland residents who have Spina Bifida, Autism, Cerebral Palsy, or other developmental disabilities and are living at home. Particularly, its self-directed nursing support is suitable for those clients who are able and wish to maintain some degree of independence and control over their care plans. CHES offers two options for its self-directed nursing support services: Nursing Consultation and Nursing Case Management with Delegation. The organization uses each patient's Health Risk Screening Tool (HRST) level, their demonstrated degree of independence, medication needs, and current caregiver situation to determine which type of support service is most suitable. To start, a Registered Nurse (RN) from CHES completes an HRST rating or review session with the patient and their family and care staff.

Nursing Consultation is best for clients with an HRST level of 3 or higher who manage their own healthcare and administer their own medications and treatments. This option is also appropriate for clients who direct and take responsibility for their paid staff to complete their care tasks, or who rely on unpaid caregivers, such as family members.

Nursing Case Management and Delegation is best for clients who require more attention and support than Nursing Consultation provides. With this option, a CHES RN works with the client to develop their care plan and then trains their staff to carry the plan out day-to-day. The RN also delegates and supervises the staff to ensure the care plan is sufficiently carried out and is "on call" during all business hours.
