The CCM program generally provides non-face-to-face services to Medicare beneficiaries who have multiple (two or more) chronic conditions expected to last at least 12 months. Patients in their hospital-at-home can expect concierge care coordination, daily health and preventative medicine assessments, a 24/7/365 clinical care team available to them, intimate health coaching and motivation, and telemedicine and remote monitoring, including psychological monitoring. Appointment reminders, care plan communication, patient education, and annual wellness visits are also included.
